The Midi USB drivers should be installed with the CD that came with the FG.
I use the FG8 with my PC (Cubase4) and get the Fantom coming up as an IN/OUT midi option together with my other Steinberg USB and M-Audio Midi ports.

Thanks for all your interest in this. For some reason the glitching is much more obvious if I put the V/O sample in the song?!?
Anyway have a play I am posting the .mid file and the VO.mp3
The V/O should go in at bar 7.
